This website is informational. It is not a patient portal and the temporary appointment form is not medical intake.

Information submitted through the form

The appointment form asks for your name, email address, optional phone number, and permission to contact you about scheduling. Do not submit symptoms, diagnoses, medication information, or other medical details.

How the information is used

Contact information is used to respond to your request and coordinate scheduling. The form sends the request to Mandy Nichols Wellness through an email-delivery provider. It is not used for advertising.

Analytics and advertising

The website does not currently use advertising pixels, session replay, or cross-site retargeting tools.

Email is not for medical care

Standard email may not be secure. Do not use email or this form for urgent concerns, medical advice, prescription requests, or emergencies. Call 911 for an emergency.
